This page is for prospective patients wanting to register with the surgery. During online registration we ask you to complete the new patient registration form and questionnaire. We then ask that you attend for a new patient medical check with us within 4 weeks to complete registration.
Please note that online applications are only accepted from SY1 addresses. If you live outside of this postcode area and wish to register please print off the registration form and questionnaire, complete each and bring them to the surgery between 2 and 6pm anyday where we will advise you further.
This practice operates the electronic transfer of records between GP practices (GP2GP).
Upon entering your registration details onto our database your electronic records will be requested from your previous surgery.
If your previous GP surgery also operates this electronic system then your computerised record will be transmitted immediately via the NHS spine and into your electronic record at this practice.
This system provides many advantages but primarily it is the speed at which we can access patient's previous medical history and medication details.
